Accidental Double Purchase creating a big $10M Loss

I purchased CHRK5 and somehow it showed that it was purchase twice (99999) at $10M each transactions.  This error resulted in a purchase price at twice its value and instanteneouly showed that I had loss of 50% resulting in a $10M loss for this morning.
