As for healthcare in Spain, it was probably universal already then. It seems you carry some political bias here, that universal health care is not good, it's sub-par? This is not the case. It's what conservatives want you to believe. They are the political arm of big business, and universal health care threatens some of their big clients/sponsors. Many countries in (western) Europe have excellent health care. The point you bring up here thus seems to be a non-issue.

Also, considering Martel, either my memory is faulty, or you are forgetting something? Lena wished for him to be dead. Unless she was an heir to his fortune, this would cut her off from that source of financial support. I don't believe she was an heir to his fortune? Thus, her being indifferent to his perceived passing (in her bed, after they made love several times in a row), kind of undercuts the theory that she was dependant on him for financial support? That is, unless my memory of some details in the film is faulty.
